Informed Consent for Teleconsultation

By proceeding to payment on MyMedicalCertificate, you acknowledge that you have read, understood, and consent to this Informed Consent Policy. This consent is obtained as required by the Telemedicine Practice Guidelines 2020 issued by the Medical Council of India (now National Medical Commission).
1. Nature of Teleconsultation
You understand and acknowledge that:
- This is a video/audio teleconsultation — the doctor will consult you remotely via Google Meet
- The doctor cannot conduct a physical examination (auscultation, palpation, etc.)
- The consultation is based on your verbal description, uploaded documents, and any visible signs during the video call
- Teleconsultation is appropriate for certificate issuance, minor ailments, and follow-up consultations
- Teleconsultation is NOT suitable for acute emergencies, complex diagnostic workups, or conditions requiring physical examination
2. Limitations of Telemedicine
You acknowledge that telemedicine has inherent limitations:
- The accuracy of diagnosis may be limited without physical examination
- Technical issues (internet connectivity, video quality) may affect consultation quality
- The doctor may refer you to an in-person consultation if they determine it is necessary
- Not all medical certificates can be issued via telemedicine — the doctor will advise
3. Patient Responsibilities
By proceeding, you confirm that you:
- Have provided truthful and accurate information about your health condition
- Have uploaded relevant supporting documents if required
- Will be available and present during the consultation at the scheduled time
- Understand that providing false information to obtain a certificate may be a criminal offence under the IPC
- Will seek in-person care immediately if your condition deteriorates or you experience an emergency
4. Data Consent
You consent to:
- Your personal and medical information being shared with the assigned doctor for consultation purposes
- Your consultation being logged (audio/video is NOT recorded; intake data and EMR notes are stored)
- Your certificate PDF being stored securely and delivered via WhatsApp and email
- Your booking reference ID being usable for certificate verification by third parties (employer, RTO, etc.) — no medical details are shared during verification, only certificate validity status
- Anonymised, aggregated usage data being used to improve our platform (no personal identification)
5. Doctor's Clinical Autonomy
The consulting doctor exercises independent clinical judgment. You understand and accept that:
- The doctor may decline to issue a certificate if, in their clinical judgment, it is not warranted
- The doctor may issue a prescription, refer you to a specialist, or recommend tests instead of or in addition to a certificate
- If the doctor declines to issue a certificate, you will receive a full refund (see Refund Policy)
- The certificate content is the doctor's professional assessment — you may not dictate the clinical findings
6. Voluntary Consent & Right to Withdraw
Your consent is voluntary. You may withdraw consent at any time before the consultation begins and receive a refund per our Refund Policy. Once the consultation is complete and the certificate is issued, the service is considered delivered.
7. Emergency Disclaimer
Emergency Notice
MyMedicalCertificate is NOT an emergency service. If you are experiencing a medical emergency — chest pain, difficulty breathing, severe bleeding, stroke symptoms, or any life-threatening condition — call 112 immediately or go to the nearest hospital.Do not use this platform for emergencies.
8. Regulatory Compliance
This consent is obtained in compliance with:
- Telemedicine Practice Guidelines 2020 (MCI/NMC, India)
- Indian Medical Council (Professional Conduct, Etiquette and Ethics) Regulations, 2002
- IT Act 2000 and IT (Amendment) Act 2008
- Digital Personal Data Protection Act (DPDPA) 2023
